OSDI Dry Eye Questionnaire

The Ocular Surface Disease Index (OSDI) is a short, 12-question survey designed to measure the severity of dry eye symptoms and how they affect your everyday life. It looks at three key areas:

  • Ocular symptoms (such as light sensitivity, grittiness, or soreness)
  • Visual function (how your eyes feel during activities like reading or computer work)
  • Environmental triggers (like wind, air conditioning, or dry air)

Your score helps to give an overall picture of your eye comfort over the past seven days. This tool doesn’t diagnose dry eye disease, but it’s a simple way to understand whether your symptoms are mild, moderate, or severe—and whether you might benefit from further assessment or treatment; and how treatments are impacting your eyes
